I..............................DO SOLEMLY,SINCERELY AND TRULY DECLARE AND AFFIRM THAT THE EVIDENCE WHICH I SHALL GIVE IN THIS CASE SHALL BE THE TRUTH, THE WHOLE TRUTH AND NOTHING BUT THE TRUTH.

Note: For those people who are unable to take oaths because of a religious conviction, they are allowed to make an affirmation instead of swearing. The penalty for perjury is  the same.
